Once the language pack is installed, AutoCAD does not automatically switch languages. You have to select it:

The language pack must match your software version exactly. You cannot install an AutoCAD 2019 language pack on AutoCAD 2018.

Before 2011, Autodesk released entirely separate installer executables for each language (e.g., AutoCAD_2018_English.exe , AutoCAD_2018_German.exe ). This was a maintenance nightmare. Each language build required its own QA cycle, patches (Service Packs), and disk images.
